The following is required for the administration of any medication including OTC medications:
*MD notation indicating medication and instructions on usage (dose, frequency must be included).
*Written permission from the parent to administer the medication.
*Medication is to be in a properly labeled container by the pharmacy (OTC medications are to be in the manufacturer's container) and is to be given to the nurse by the parent/guardian.
*Student commitment to obtain the medication from the nurse.
*Please read Form #300, and then fill out Forms #301, #302, and Provider and Parent Permission Form for self medication (Form #304) as appropriate. Proper documentation is required by New York State laws for medication to be taken in school.
