As required by New York State, students must provide proof of having received the required vaccines at the appropriate intervals.
A STUDENT IS CONSIDERED IN PROCESS IF HE/SHE HAS AT LEAST THE FIRST DOSE OF EACH IMMUNIZATION SERIES AND HAS AGE APPROPRIATE APPOINTMENTS TO COMPLETE THE IMMUNIZATION SERIES. ACIP CATCH UP SCHEDULE IS USED TO DETERMINE APPROPRIATE SPACING OF APPOINTMENTS.
The immunizations may be documented on the form "Immunization Record" (Form #200) or on the physician’s office form.
NEW requirements affecting all students entering 12th grade!
All students entering 12th grade must be properly immunized with the meningococcal vaccine (Men ACWY). A dose must be received on or after the 16th birthday regardless of the number of doses received before 16 years of age.
Medical exemptions must be reissued annually; and must specify the immunization exempted, provide sufficient information to identify the medical contraindication to the specific immunization, and specify the length of time the immunization is medically contraindicated.
*Demonstrated serologic evidence of polio, measles, mumps, rubella, hepatitis B or varicella antibodies is acceptable proof of immunity to these diseases. Diagnosis by a physician, physician assistant or nurse practitioner that a child/student has had measles, mumps, or varicella diseases is acceptable proof of immunity to those diseases.
